The origins of the cathedral and its diocese are uncertain, but it is known that they existed before the year 1000, closely linked to the Lombard diocese of Benevento. Historical documentation begins at the end of the 10th century when Bishop Giovanni rebuilt or restored the ancient cathedral in the form we know today. The cathedral underwent further work under Bishops Roberto and Pietro between 1188 and 1231, transforming it into a Romanesque style. During the Baroque period, the church's interior was transformed according to contemporary tastes. In 1930, the cathedral was destroyed by an earthquake and rebuilt as it was, with the exception of the Baroque additions, which were largely eliminated to restore its original Romanesque appearance. The church was reconsecrated in October 1936. The cathedral was declared a national monument in 1890 and elevated to the dignity of a minor basilica by Pope Paul VI on June 10, 1970.
